Six of nine disagreements this quarter came down to scope rather than capability. The requirement now reads 'multi-site' where it previously read 'operational leadership', and the mismatch has stopped recurring.

Outcomes are recorded by a person, not inferred. A rejection at panel is not treated as an error in the assessment — it is evidence about which requirements actually mattered to that client.
